TruForm Classic: Archiving a TruForm Submission

Instructions below ONLY apply to TruForm on MyPBHS.

How to Archive a Registration Submission

*Please note our truform submission storage details here.  

  1. Login to the MyPBHS Portal  
  2. Scroll down and select the "Registrations" option (located towards the middle of this page). 
    • Note the additional option located at the top under the "TruForm" tab labeled "View Registrations"- this will also lead you to your Registration submissions.

  3. Find the patient submission you would like to archive within your "Patient Registrations" list
  4. Select "Archive" to the far RIGHT of the patient name

  5. The submission will be removed from this list and added to your archives under "Recovery Patient Registrations".  
    *From here you will have up to 90 days to recover the form needed from it's submission date before it is permanently removed from our servers.

        

How to Archive a Referral Submission

  1. Login to the MyPBHS Portal  
  2. Scroll down and select the "Referrals" option (located towards the middle of this page).
    • Note the additional option located at the top under the "TruForm" tab labeled "View Referrals"- this will also lead you to your Referral submissions.

  3. Find the referral submission you would like to archive within your "Doctor Referrals" list
  4. Select "Archive" to the far RIGHT of the patient name

  5. The submission will be removed from this list and added to your archives under "Recovery Doctor Referrals".  
    *From here you will have up to 90 days to recover the submission before it is permanently removed from our servers.
