TruForm Classic: Recovering an Archived TruForm Submission

Instructions below ONLY apply to TruForm on MyPBHS.

Once you archive a registration or referral submission, you will have up to 90 days to recover this same submission back (to view, print or save) within your MyPBHS Portal.

How to Recover an Archived Registration Submission

  1. Login to the MyPBHS Portal  
  2. On your far right hand column, locate the section labeled "Recover Files"
  3. Select "Recover Patient Registrations"

  4. Within your "Recover Patient Registrations" section, you can search for the patient submission by typing in the first and/or last name of the patient, OR use the page icons to search by page.

    *You can permanently REMOVE the submission from our servers by selecting the "Delete" option.  PLEASE NOTE THERE IS NO WAY TO RECOVER THIS SUBMISSION BACK ONCE YOU DELETE IT FROM THE RECOVERY AREA!!!
        
  5. Once you find the patient submission, select the "Recover" option to the RIGHT of the patient name:

  6. Stay on this page until you see a confirmation this has been done (the confirmation will be directly below the title "Recover Patient Registrations"), as seen below
  7. Now select the "View Registrations" located right below the "TruForm" tab

  8. You should notice the submission you recovered is at the top of your list to view, print or save as needed:

How to Recover an Archived Referral Submission

  1. Login to the MyPBHS Portal  
  2. On your far right hand column, locate the section labeled "Recover Files"
  3. Select "Recover Doctor Referrals"

  4. Within your "Recover Doctor Referrals" section, you can search for the doctor referral submission by typing in the first and/or last name of the patient, OR use the page icons to search by page.

  5. Once you find the doctor referral submission, select the "Recover" option to the RIGHT of the patient name:

  6. Stay on this page until you see a confirmation this has been done (the confirmation will be directly below the title "Recover Doctor Referrals"), as seen below
  7. Now select the "View Referrals" located right below the "TruForm" tab

  8. You should notice the submission you recovered is at the top of your list to view, print or save as needed:
