TruForm Classic: Form Options

Instructions below ONLY apply to TruForm on MyPBHS.

In this section we will go over the following:


TruForm Classic - First Time Setup

At the time of setup you will have the option to use our standard forms (as-is) OR you can email your personal forms in either a word document or a PDF format.  

You can choose to use our standard form or your personal form:

TruForm v.5.0   |  Oral Surgeons Form Option ONLY

  • TruForm v. 5.0 is iPad responsive. 
  • This option is only available with our standard OMS TruForm
  • This standard form cannot be customized in any way!  **If you do want to customize our standard OMS form, you can use and make customizations to TruForm v. 3.0.

OMS FORM EXAMPLE:

  • [Preview]  Your patients would fill out this form from your website
  • [Preview]  Once submitted, your office would receive this completed PDF.  

SIGNATURE FEATURES:

  1. ALL signatures can be signed by the patient BEFORE the form is submitted. 
  2. They can do this from ANY iPad/Tablet device OR from a desktop computer using their mouse to sign!!

**ONCE TRUFORM IS SUBMITTED, YOU CAN NO LONGER FINISH SIGNING ON AN iPAD.   Once submitted, any signatures left unsigned will be programmed to be signed using a topaz signature pad within adobe acrobat.

TruForm v.3.0 | OMS, Endo, Perio, & GD Form Options

REGISTRATION OPTIONS for v.3.0:

Registration (OMS): 

If you wish to use our standard OMS truform with customizations, you can accomplish this by using our TruForm v.3.0 OMS Form below.  If you don't wish to make customizations to this form, you would use our TruForm v.5.0 option above.

Registration (Endo, Perio & GP Only):

  • [Preview]  Your patients would fill out this form from your website
  • [Preview]  Once submitted, your office would receive this completed PDF.

REFERRAL OPTIONS for v.3.0:

Referral (OMS):

  • [Preview] Your referring doctors would fill out this form from your website
  • [Preview] Once submitted, your office would receive this completed PDF

Referral (ENDO):

  • [Preview] Your referring doctors would fill out this form from your website
  • [Preview] Once submitted, your office would receive this completed PDF

Referral (PERIO):

Health History Update Form (OMS, PERIO, ENDO, GD):

For returning patients, we have a standard health history update form. 

SIGNATURE FEATURES:

  1. ALL signatures can be signed by the patient AFTER the form is submitted, in your office. 
  2. ALL signatures will be programmed to be signed using a topaz signature pad within adobe acrobat. 

Customizing truForm v.3.0

Are you a New or Existing Client?

NEW CLIENTS:
When you sign up for TruForm, our support department will send you out setup questions via email to start this process.  You will have the option to submit your personal form or use our standard forms (as-is or with further changes):

Personal Forms PBHS Standard Form
  • Email personal form(s) to support@pbhs.com in either a PDF or Word Document format.
  • Your form will be programmed over the design as-is, this will allow patients to fill out this exact document online.
  • The final product will be programmed and added within your TruForm Account 
  • Email form changes to support@pbhs.com
  • Our form designer will contact you to go over changes and pricing ($135/an hour).
  • A proposal will need to be authorized by your office before any work is completed
  • The final design will be programmed and added within your TruForm Account.

EXISTING CLIENT:
I am existing truform client, and forms are live on my account.  I would now like to make further changes to my forms.  Depending on the type of TruForm you are using (either personal or standard), follow the steps below to submit your changes!

Personal Forms PBHS Standard Form
  • Email personal form(s) to support@pbhs.com in either a PDF or Word Document format.
  • Out support department will contact you to go over changes and pricing ($150 per page)
  • A proposal will need to be authorized by your office before any work is completed
  • The final design will be programmed and added within your TruForm Account.
  • Email form changes to support@pbhs.com
  • Our form designer will contact you to go over changes and pricing ($135/an hour for design & $150/per page to reprogram the form).
  • A proposal will need to be authorized by your office before any work is completed
  • The final design will be programmed by support and added within your TruForm Account.
