truForm: Introduction

Instructions below ONLY apply to truForm.

We are pleased to inform you that we have upgraded our truForm system to a better, more secure, and more synergized platform.


The basics

As a current user of our truForm service, (if you do not use any software integration we provide) PBHS Support will upgrade your truForm account to be part of our new HIPAA compliant portal (https://mysecurepractice.com). The general process remains the same:

  1. TruForm will be placed on your website for your patients and/or referring doctors to submit forms securely to your office.
  2. Each time a patient or doctor submits your online form, your office will receive a notification email.  This email will provide a link to your login page here:  https://mysecurepractice.com
  3. You will create your own login used to view/print and save your online form submissions.

How does it work?

Our new truForm system is HIPAA compliant and built to be mobile compatible and designed to work with any PBHS standard or custom form provided. With your custom PDF form, we can create a matching optimized HTML "front end" form that will be completed by the patient or referring doctor. Once submitted, all information is transferred onto your PDF form for your review.

For example, the two forms below demonstrate our PBHS standard OMS form built into the new truForm system.

It is important to understand we program these forms to match word for word, with the exact same questions given on the PDF form provided (either PBHS standard or your custom forms).


How to complete an online patient registration form
*For patients registering online from your website*

1. When you click on the link to the truForm online registration from the website you will be presented with an secure, interactive online form:

2. Fill out the form completely, some fields (including the patient name) will be required for submission.

3. If you'd like, at any time during the completion of the form you may click the "download" button in the upper right hand corner.

This will download a PDF file of the form with any fields you have already completed filled out. It may look something like this:

4. Once you have fully completed the form, you can click download to download and save or print a copy for your records.

5. When you are ready to send the registration form securely, use the "Complete and Send" button in the bottom right hand corner of the screen.

This will securely send the online registration form to the office for review!

6. After the form is submitted you are prompted to attach files of your choice - this could be scans of insurance cards, x-rays, forms or any other documents (up to 100MB).

You will see these attachments at the bottom of the upload window to review before submission. To fully submit the registration form with attachments, click Finish.

8. Your secure online registration submission is complete when you see the "thank you" message confirmation!

  • From this screen you also have the button option to Download Submitted Form and this will provide you a PDF copy of your submitted form for your records.

 


How to complete an online doctor referral form
*For your referring doctors sending you online referrals from your website*

1. When you click on the link to the truForm from the website you will be presented with an secure, interactive online form:

2. Fill out the form completely, some fields (including the patient name) will be required for submission.

3. If you'd like, at any time during the completion of the form you may click the "download" button in the upper right hand corner.

This will download a PDF file of the form with any fields you have already completed filled out. It may look something like this:

5. When you are ready to send the referral securely, use the "Complete and Send" button in the bottom right hand corner of the screen.

This will securely send the online referral to your colleague's office for review!

6. After the form is submitted you are prompted to attach files of your choice - this could be patient x-rays, forms or any other documents (up to 100MB).

You will see these attachments at the bottom of the upload window to review before submission. To fully submit the referral with attachments, click Finish.

7. Your referral submission is complete when you see the "thank you" message confirmation!

  • From this screen you also have the button option to Download Submitted Form and this will provide you a PDF copy of your submitted form for your or the patient's records.

 


Tips for submitting forms within the new truForm system

Here are a few tips to assist your office with online truForm submissions:

Downloading a PDF of your completed form:
Once you have filled in the required fields (first name, last name, phone number and email) you have the ability to download the form for your own records! Simply click the “Download” button in the top right corner of the form and you should see a PDF of the form downloaded to wherever files from the internet are saved.

Attaching X-Rays and other information: You are able to attach most file formats to our referral form. Once the form fields are completed , please click “complete and send”. Once submitted, you will have the option to attach files. Our system supports all industry standard imaging formats for x-rays. If you have trouble attaching any files, feel free to contact the PBHS Support Department at 888-840-0739  and they can address your needs accordingly. They are open M – F from 5 AM to 5:30 PM PST.


Benefits of the new truForm system

Some of the benefits of our truForm system include:

  1. Mobile Compatible Forms
    This will allow anyone who uses your form(s) to view them from any platform (including PC, MAC, Tablets, and Smart Phones).  
  2. Digital Signatures (before the form is submitted to your office)
    Our forms now allow for digital signature fields. We are able to build a special "optimized" front-end form to match any custom registration or referral form.
  3. Digital Promotion with your Patients:
    Your staff can now invite patients to complete their forms prior to their office appointment via email. 

Software integration with the new truForm system

Software integration on our truForm system is currently available only for EDDSON, Henry Schein, and Medims clients. If you are currently using another practice management software, the integration is still under development and will become available in the near future.
