Instructions below ONLY apply to truForm with Eaglesoft.
The truForm integration requires no additional charge from PBHS, and can be setup at anytime with your PBHS truForm order. Please contact PBHS Support further if you are ready to start this process.
In order to complete this section, you must have the following completed:
- Checking Your Eaglesoft Software Version
- Minimum Technical Specifications
- Creating truForm Integration Credentials
- Adding Integration Credentials Into Eaglesoft
Once these 4 steps above are complete, please proceed below:
- Navigate to your desktop and double click on the desktop icon labeled, truForm Bridge:
- Select the Check Now button to check and pull in any new truForm submissions:
- A pop up window will give you the total number of new truForm submissions downloaded, select the OK button to proceed:
- Now select the Start Wizard button:
- Within the wizard, you will see the first patient to integrate:
- You can choose to:
Merge to Existing Patient
Our truForm Bridge will always check each truForm submission for any potential matches through Eaglesoft. If there are any potential matches, you will have the option to merge this new patient submission into their existing account. Our truForm wizard will walk you through each screen; displaying the original account information to the right, and the newly submitted truForm data to the right for review.
**Please note we do not integrate Responsible Party Information for any Existing Patients within Eaglesoft!
- Within the truForm Bridge Wizard, any Potential Matches within Eaglesoft will display in a list to your RIGHT:
- Select the option labeled Merge to Existing patient, and select the matching patient name in the list to your RIGHT.
- Select the Next button to begin:

- On the Patient Demographics screen: you can review the original patient record to the LEFT, and the modified patient record with the updated truForm information to the RIGHT.
- Select the Next button to continue:

- On the Health History screen: you can review the original patient record to the LEFT, and the modified patient record with the updated truForm information to the RIGHT.
- Select the Next button to continue:

- On the Emergency Information screen: you can review the original patient record to the LEFT, and the modified patient record with the updated truForm information to the RIGHT.
- Select the Merge button to merge the data reviewed into the existing patient account:

- A pop-up confirmation will ask you to confirm this action, select the Yes button.
- On the confirmation page, it will tell you if you have any additional patients to integrate.
- From here you can decide if you are done by selecting the Finish button:
- If you select the Continue button to integrate the next patient, you will have the following options again:

Create a New Patient
- Within the truForm Bridge Wizard, select the option labeled Create a new patient:
- Select the Next button to begin!
- On the Patient Demographics screen, you can review the integrated fields to the RIGHT and select the Next button after review:
- On the Responsible Party screen, you can review the integrated fields to the RIGHT and select the Next button after review:
**The truform bridge will integrate Responsible party for New Users!
- On the Health History screen, you can review the integrated fields to the RIGHT and select the Next button after review:
- On the Medications / Allergies screen, you can review the integrated fields to the RIGHT and select the Next button after review:
- On the last Emergency Information screen, you can review the integrated fields to the RIGHT and select the Merge button to merge this information into a new account:
- Confirm the new account creation by selecting the YES button:
- On the confirmation page, it will additionally tell you if you have additional patients to integrate. From here you can decide if you are done by selecting the Finish button:
- If you select the Continue button to integrate the next patient, you will have the following options again:
Within your Eaglesoft software, the patient record will be saved:
**Please note, you may need to close and reopen this 'Person List' window in order to see the patient truForm you just integrated.
Discard truForm Submission
This will remove the truForm submission so it does not continue to try and integrate within Eaglesoft. *Please note, once you confirm to discard the truForm submission, this action cannot be undone.
- Within the truForm Bridge Wizard, select the option labeled Discard truForm Submission:
- Select the Next button
- A pop-up will ask you to confirm this action, as it cannot be undone. Select the YES button to remove this truForm submission:
- or you can select the NO button to cancel this request.
- On the confirmation page, select the Finish button:
- If you have any new patients, you would see a screen similar to this:

- This will bring you back to your truForm Bridge:
