Converting truForm into Eaglesoft from Portal

In order to complete this section, you must have the following completed:

  1. Checking Your Eaglesoft Software Version
  2. Minimum Technical Specifications
  3. Setup Of The PBHS truForm Bridge
  4. Creating truForm Integration Credentials
  5. Adding Integration Credentials Into Eaglesoft

Once these 5 steps above are complete, please proceed below:

  1. Login to the MySecurePractice Portal
  2. Select the Access Forms icon

      • OR - Once off the main welcome dashboard, you will select Forms on your far LEFT hand side menu.

  3. On the LEFT hand side menu, select the Truform Bridge option:

  4. Under the Submissions Overview table, find the patient you would like to convert:

    • You can use the Search Forms field to search for that patient by either first name or last name, once entered select the green magnify icon to complete your search:

    • To clear the search results, remove the text added in the Search Forms field- and select the the magnify icon to reset your Submission Overview list:

  5. Select the Integrate button:

    1. If you DO NOT wish to integrate the truForm at all, select the Integrate drop down button:

    2. Then select the Discard option:

    3. Select the OK button to confirm this action:

    4. Back on the truForm Bridge dashboard, a confirmation will appear as well as the removal of the patient truForm from the Submissions Overview table:

  6. If you decide to Integrate the patient truForm, you can now decide if you would like to:
    1. Create a new patient
    2. Merge with selected patient (if there is a potential match)

Merge with Selected Patient

Our truForm Bridge will always check each truForm submission for any potential matches through Eaglesoft.  If there are any potential matches, you will have the option to merge this new patient submission into their existing account.  Our truForm wizard will walk you through each screen; displaying the original account information to the right, and the newly submitted truForm data to the right for review.

Any updated items that will be changed in the patient's account, will be highlighted in yellow under the After Merge table.

**Please note we do not integrate Responsible Party Information for any Existing Patients within Eaglesoft!   

  1. Once you've selected the Integrate button for the selected patient truForm
  2. If there is a potential match, select the patient under the Potential Matches table:

  3. Select the Merge with selected patient button:

  4. Once selected, select the Continue button:

  5. Here you can verify the Patient Demographics information provided:before-after.png
    1. Before Merge Column:  What is currently in the Eaglesoft Patient Account
    2. After Merge Column:  Updated information from the Patient's new truForm submission
      • Any changes to existing fields will be highlighted in yellow, so you can easily see what values will be updated within your software:
      • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  6. On the Responsible Party section, you will be reminded that the responsible party cannot integrate for existing patients within Eaglesoft:

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  7. Verify the Health History Information provided:

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  8. Verify the Medications/Allergies information provided:

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  9. Verify the Emergency Information provided:

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  10. Select the OK button to finish processing the patient truForm:

  11. Back on the TruForm Bridge dashboard, you will see a confirmation that the patient truForm has been processed:

    1. Once processed, that patient will be added to the Integrated Submissions Overview table. 
      **This table will include truForms that have been processed and will be pulled into your practice management software at the next sync interval:

Create a New Patient

  1. Once you've selected the Integrate button for the selected patient truForm
  2. If there are no Potential Matches, select the Create a new patient button:

  3. Select the Continue button to proceed:

  4. Verify the Patient Demographics information provided:
    **please note there will be NO Before Merge since this is a brand new patient not already in your Eaglesoft software.

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  5. Verify the Responsible Party information provided, which will integrate for brand new users ONLY (not already in your Eaglesoft software):

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  6. Verify the Health History information provided:

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  7. Verify the Medications/Allergies information provided:

    • Once reviewed, scroll down to the bottom of this page and select the Continue button:

  8. Verify the Emergency Information provided, and scroll down to select the Finish button:

  9. Select the OK button to finish processing the patient truForm:

  10. Back on the TruForm Bridge dashboard, you will see a confirmation that the patient truForm has been processed:
